In the open form part of the Contact Us submission you are able to put in any information you would you like. We have no way of knowing in advance what sensitive information you place in that area and would recommend not including sensitive information such as other individual’s names or address. We strongly recommend not disclosing any information that would be covered by HIPPA in this form.
